What a biological-age test buys you

Four contrasting offerings show why the assay, algorithm, report, advice service and data contract should never be treated as one validated claim.

A dated comparison, not an endorsement. These four contrasting offerings were reviewed on 28 September 2026, without checkout, account creation, sample submission or seller contact. Displayed US-facing dollar prices are not delivered quotes. A readable listing does not establish current fulfillment; an unknown term is not evidence that the seller has no policy.

Measurement → inferenceAssay → algorithm → report

The assay measures a specimen. The model estimates a target. The report selects and labels outputs. A protein- or organ-looking result can be a prediction, not a direct measurement.

Service → evidenceAdvice and terms are separate

A consultation, membership or action programme adds a service. A model publication does not establish that the purchased package improves decisions or health.

Ownership is not assay identity. Tally’s policy states that IE Tally Holdings, LLC, a TruDiagnostic affiliate, acquired the brand/business assets on 19 March 2026; the policy is effective 29 April 2026. This is a newly documented earlier event, not a September acquisition. Two brands therefore do not establish independent-provider confirmation, but affiliation does not prove identical specimens, laboratories, weights or results. [C09]

The shop-linked Advanced TruAge sample has a June 2025 report date. It illustrates a report format, not the exact version supplied by a September 2026 purchase. No sample identity or personal interpretation is reproduced here. [F07]

A publication is not automatically validation of the whole product

TruAge: OMICmAge’s version of record appeared on 25 February 2026; Systems Age’s on 15 September 2025. Neither should be described here as merely a preprint. OMICmAge discloses sponsored research, company involvement and patent interests. External-cohort evidence is not the same as an independent commercial evaluation. Deployment uses DNAm-derived inputs, not direct measurement of the customer’s entire proteome and metabolome. [M21; M22]

TallyAge: the later CheekAge mortality analysis used a blood adaptation with incomplete features and standardized delta scores. It is relevant prognostic evidence but not proof that one current consumer cheek swab forecasts an individual death date or that the membership causes benefit. The researchers’ commercial affiliation must remain visible. [M23; M24]

GlycanAge: a small longitudinal subset in the original research had heterogeneous trajectories. That does not supply a universal current-service retest response or error bound. Seller claims about triplicates or a one-year margin need to identify whether they concern assay precision, age prediction or individual change. [M12; C04]

InnerAge: an InsideTracker-associated 2018 study followed 1,032 repeat-testing customers and examined laboratory changes, not validation of the current proprietary InnerAge 2.0 score. No tested intervention associations survived its multiple-testing correction. Observational limits remain; the linked correction notice’s text was not obtained, so its substance is not asserted. That is a lower acquisition burden, not proof that the score adds useful information beyond the laboratory results already held.

Two unchanged standalone TruAge prices total $998; two TallyAge prices total $498, before unverified extras. These sums price neither a subscription nor an evidence-based retest schedule. Include collection, shipping, taxes, interpretation, renewals and failed-sample costs; no trustworthy all-in four-way price comparison is available. [C02; C08]

Data access, retention and consent

Ask about five different objects: the physical sample, raw assay files, processed measurements, derived scores/reports, and research or aggregated derivatives. Account deletion, research withdrawal and marketing opt-out need not affect the same objects. A PDF of age numbers does not reproduce the assay-to-score calculation.

“Does not sell” is not “does not share.” Unread policy content is not proof of absent protections, and disclosure is not verification of operations. No account, consent dialog, export or deletion mechanism was tested. Research reuse needs usable raw/processed data, assay and model metadata, and permission—not just a downloadable report.

Ask what precision means for the report you will receive

CheekAge has substantive paired-collection evidence. The developer-affiliated paper reports 190 replicate pairs from two cheek collections within 24 hours, with mean replicate error of approximately 0.85 years for the full model and a separate cross-validated value. This is not development age-prediction MAE, a 95% distribution of intervisit differences, or independent verification of the current TallyAge service. It cannot supply a personal meaningful-change threshold. [M23]

OMICmAge’s reported 0.998 ICC using 30 replicates likewise does not supply a collection-to-report personal-change interval. TruDiagnostic describes multiple platforms and a paired reference resource, so “no bridging evidence” would overstate the gap. The unresolved question is a frozen, output-level bridge and absolute residual error applicable to the old and new reports being compared. [M21; F06]

A split after DNA processing covers fewer stages than two complete collections. GlycanAge’s triplicate/one-year-margin language and a seller’s same-sample reproducibility percentage need their metric, coverage and split point specified; neither converts itself into score-year intervisit uncertainty. Illustrations and improvement percentages are not controlled outcomes

TruAge’s provider page labels its before/after profiles illustrative, not actual patient results. Tally advertises that over 62% of members lowered epigenetic age within 12 months, averaging more than two years. InnerAge’s listing describes improvement among initially high-scoring customers, while current Ultimate copy uses a broader customer percentage. The inspected record does not establish common denominators, attrition, version continuity or untreated comparison groups; these are not interchangeable populations. [C01; C08; C10; F03]

Selection for a high first score, selective retesting, concurrent behavior changes, transient physiology, changing inputs and regression to the mean can affect before/after summaries. Their contribution was not quantified here. Ask for the eligible and retested denominators, fixed pipeline, comparison group and uncertainty before interpreting a customer percentage as benefit. The CALERIE group result does not supply an individual customer’s counterfactual.

These are proposed questions derived from the replicate and platform evidence, not claims that a named seller uses the studied pipeline. The same-DNA platform example is not a conversion for purchased reports. Clinical-laboratory and proteomic change thresholds additionally need score-level covariance or full-pipeline repeated predictions; individual analyte CVs or protein ICCs do not supply them. Before paying—or paying again

  1. Define the decision.

    What will the report add beyond information you already have? General advice available without testing does not itself establish the test’s value.

  2. Ask for the exact output and evidence.

    Which assay, model/version, actual input subset and report are delivered? Has a historical report been reprocessed? Does the validation use that specimen and full pipeline in a relevant population?

  3. Ask what a repeat can resolve.

    Use the precision and bridging questions above. Ask for uncertainty in the actual reported units, not a laboratory badge, age correlation or suggested retest interval.

  4. Check the whole commitment.

    Confirm the base package plus add-on, renewal, eligibility, delivered cost, failed-sample remedies and cancellation. Ask separately about sample disposal, raw files/metadata, processed data, reports and research derivatives.

Bottom line: none of the inspected material establishes long-term outcome benefit from the purchased testing-and-advice package versus a realistic no-test alternative. That is not a finding of failure or an allegation against a seller. It means “no additional test for this decision” remains a reasonable option. For a report already in hand, start with interpretation rather than score optimization.

Source labels distinguish primary research, seller documents and implementation notes. Seller and F-source observations are from 28 September 2026 unless marked historical. M21/M23/M24 received the described follow-up inspection; other M observations remain inherited from 21 September, and R measurement sources from 22 September.
